How does Case Management work?

It starts with understanding you—your health, your goals and what matters most. Your Case Manager works with you and your care team to coordinate care, help you understand treatment options, manage insurance details and connect with the right resources at the right time.

Easy steps to getting support.

After a hospitalization or illness, a WPS Case Manager may contact you by phone and mail to offer support.

What if you need a new treatment or procedure?

Nurse Navigation can help you find your way forward.

When you’re facing a specific healthcare need, decision or new treatment, you may have a lot of questions. Where do you go? What should you expect? What should you ask your doctor? Having a Nurse Navigator in your corner who works in the system can make the experience feel a little less overwhelming.
